We are seeking a compassionate Behavioral Health Care Coordinator to provide support and coordination of care for patients with mental health and substance abuse disorders. The Behavioral Health Care Coordinator will be responsible for assessing patient needs, developing care plans, and connecting patients with appropriate resources and services.

Key Responsibilities:
* Conduct patient assessments to identify behavioral health needs.
* Develop and implement care plans in collaboration with patients and providers.
* Coordinate referrals to mental health and substance abuse services.
* Monitor patient progress and provide ongoing support.
* Document care coordination activities accurately and timely.

Required Skills:
* Bachelor's degree in social work, psychology, or related field.
* Experience in behavioral health or social services.
* Strong assessment and crisis intervention skills.
