We are seeking a dedicated and compassionate Behavioral Health Care Coordinator to provide support and coordination of care for patients with behavioral health needs. The ideal candidate will be responsible for assessing patient needs, developing care plans, and connecting patients with appropriate resources. Key Responsibilities:
