Overview

We are looking for a skilled Clinical Pharmacist to provide comprehensive pharmaceutical care to patients. The Clinical Pharmacist will work closely with physicians and other healthcare professionals to optimize medication therapies, prevent drug-related problems, and improve patient outcomes. This role requires a strong clinical background, excellent communication skills, and a commitment to evidence-based practice.

Key Responsibilities

• Review and evaluate patient medication profiles for appropriateness and safety.
• Provide drug information and education to healthcare providers and patients.
• Participate in multidisciplinary rounds and patient care conferences.
• Monitor and manage adverse drug reactions and medication errors.
• Develop and implement clinical pharmacy programs and initiatives.

Required Skills

• Doctor of Pharmacy (PharmD) degree.
• Residency or fellowship training in clinical pharmacy.
• Board certification in a relevant specialty area.
