Job Overview

We are seeking a dedicated Field Clinical Care Coordinator to provide comprehensive support and coordination for patients in their homes or community settings. The Field Clinical Care Coordinator will work closely with patients, families, and healthcare providers to ensure seamless delivery of care, promote patient well-being, and optimize health outcomes. Key Responsibilities

• Conduct in-home or community-based assessments to identify patient needs and develop individualized care plans.
• Coordinate and schedule medical appointments, transportation, and other necessary services for patients.
• Monitor patient progress and adherence to treatment plans, and provide ongoing support and education.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care delivery.
