We are seeking a dedicated Home Health Speech Language Pathologist (SLP) to provide essential therapy services to patients in their homes. This role offers the opportunity to improve patients' communication and swallowing abilities, enhancing their overall quality of life.

Key Responsibilities:

• Conduct comprehensive patient assessments to identify communication and swallowing disorders.
• Develop and implement individualized treatment plans tailored to each patient's needs.
• Provide therapy to improve speech, language, voice, and swallowing skills.
• Collaborate with other healthcare professionals to ensure coordinated and effective patient care.

Required Skills:

• Active Oregon SLP License
• Experience with Oasis Documentation
• Strong communication and interpersonal skills
