Job Overview
We are seeking a dedicated and experienced Lead Case Manager to oversee and coordinate patient care within our Tampa, FL facility. The Lead Case Manager will be responsible for ensuring the delivery of high-quality, efficient, and cost-effective healthcare services. This role requires strong leadership skills and a commitment to improving patient outcomes.
Key Responsibilities
- Manage and supervise a team of case managers, providing guidance and support.
- Develop and implement case management plans in collaboration with healthcare providers.
- Monitor patient progress and adjust care plans as needed.
- Ensure compliance with regulatory requirements and organizational policies.
- Serve as a liaison between patients, families, and healthcare team members.
Required Skills
- RN or Social Work licensure.
- Minimum of 2 years’ case management or social work experience.
- Leadership or supervisory experience.
- Excellent communication and interpersonal skills.
