We are seeking a highly motivated, compassionate, and skilled Nurse Practitioner to join our dynamic team. The successful candidate will play a pivotal role in delivering high-quality primary healthcare services, working autonomously and in collaboration with our interdisciplinary team of physicians, registered nurses, and administrative staff. KEY RESPONSIBILITIES:

• Provide comprehensive primary healthcare services to patients of all ages, including assessment, diagnosis, treatment, and management of acute and chronic conditions.
• Conduct thorough physical examinations, order and interpret diagnostic tests, and develop individualized care plans.
• Prescribe medications and other therapies in accordance with provincial regulations and best practice guidelines.
• Provide health education, counseling, and preventative care, promoting healthy lifestyles and disease prevention.
• Manage complex patient cases, including those with multiple comorbidities, in collaboration with other healthcare professionals.
• Refer patients to specialists and other healthcare services as needed.
• Maintain accurate and timely electronic health records (EHR).
• Participate in quality improvement initiatives and continuous professional development.
• Collaborate effectively with physicians, nurses, and other healthcare team members to ensure seamless patient care.
• Provide leadership and mentorship to junior staff and students, as appropriate.

QUALIFICATIONS:

• Current registration as a Registered Nurse in the Extended Class (NP) with the College of Nurses of Ontario (CNO).
• Master of Science in Nursing (MScN) or equivalent Nurse Practitioner program.
• Minimum of 2-3 years of experience as a Nurse Practitioner, preferably in a family health or primary care setting.
• Demonstrated expertise in assessment, diagnosis, treatment, and management of a wide range of health conditions across the lifespan.
• Strong critical thinking, problem-solving, and decision-making skills.
• Excellent communication, interpersonal, and collaboration skills.
• Ability to work effectively both independently and as part of an interdisciplinary team.
• Proficiency in using electronic health records (EHR) systems.
• Commitment to patient-centered care and continuous learning.
