Job Description

<p>We are seeking a highly motivated, compassionate, and skilled Nurse Practitioner to join our dynamic team. The successful candidate will play a pivotal role in delivering high-quality primary healthcare services, working autonomously and in collaboration with our interdisciplinary team of physicians, registered nurses, and administrative staff. This is an exciting opportunity for an NP who is passionate about family health and eager to contribute to a vibrant practice in a growing community.</p><h3>KEY RESPONSIBILITIES:</h3><ul><li>Provide comprehensive primary healthcare services to patients of all ages, including assessment, diagnosis, treatment, and management of acute and chronic conditions.</li><li>Conduct thorough physical examinations, order and interpret diagnostic tests, and develop individualized care plans.</li><li>Prescribe medications and other therapies in accordance with provincial regulations and best practice guidelines.</li><li>Provide health education, counseling, and preventative care, promoting healthy lifestyles and disease prevention.</li><li>Manage complex patient cases, including those with multiple comorbidities, in collaboration with other healthcare professionals.</li><li>Refer patients to specialists and other healthcare services as needed.</li><li>Maintain accurate and timely electronic health records (EHR).</li><li>Participate in quality improvement initiatives and continuous professional development.</li><li>Collaborate effectively with physicians, nurses, and other healthcare team members to ensure seamless patient care.</li><li>Provide leadership and mentorship to junior staff and students, as appropriate.</li></ul><h3>QUALIFICATIONS:</h3><ul><li>Current registration as a Registered Nurse in the Extended Class (NP) with the College of Nurses of Ontario (CNO).</li><li>Master of Science in Nursing (MScN) or equivalent Nurse Practitioner program.</li><li>Minimum of 2-3 years of experience as a Nurse Practitioner, preferably in a family health or primary care setting.</li><li>Demonstrated expertise in assessment, diagnosis, treatment, and management of a wide range of health conditions across the lifespan.</li><li>Strong critical thinking, problem-solving, and decision-making skills.</li><li>Excellent communication, interpersonal, and collaboration skills.</li><li>Ability to work effectively both independently and as part of an interdisciplinary team.</li><li>Proficiency in using electronic health records (EHR) systems.</li><li>Commitment to patient-centered care and continuous learning.</li></ul>
