We are seeking a dedicated and experienced Telephonic Case Manager RN to provide remote healthcare support and coordination to patients. The Telephonic Case Manager RN will assess patient needs, develop care plans, and facilitate communication between patients and healthcare providers.
Key Responsibilities:
* Conducting telephonic assessments of patient health status.
* Developing and implementing individualized care plans.
* Coordinating referrals to specialists and community resources.
* Providing education and support to patients and their families.
* Monitoring patient progress and adjusting care plans as needed.
Required Skills:
* Valid RN license.
* Experience in case management or telehealth.
* Excellent communication and interpersonal skills.
* Proficiency in electronic health records (EHR).
