We are seeking a dedicated and experienced Telephonic Case Manager RN to provide remote care coordination services to our patients. The Telephonic Case Manager RN will assess patient needs, develop care plans, and provide ongoing support to ensure optimal health outcomes.

Key Responsibilities:
* Conduct telephonic assessments of patient health status.
* Develop and implement individualized care plans.
* Provide education and support to patients and their families.
* Coordinate with healthcare providers to ensure continuity of care.
* Monitor patient progress and adjust care plans as needed.

Required Skills:
* Valid RN license.
* Experience in case management.
* Excellent communication and interpersonal skills.
