We are seeking a dedicated and organized Case Manager to provide comprehensive support and advocacy for our clients. The Case Manager will assess client needs, develop and implement care plans, coordinate services, and monitor client progress. The ideal candidate will have a strong background in social work or a related field, excellent communication skills, and a commitment to helping individuals achieve their goals.
