Job Overview

We are seeking a dedicated and skilled Home Health Speech Language Pathologist (SLP) to provide essential therapy services to patients in their homes. This role involves assessing, diagnosing, and treating speech, language, communication, and swallowing disorders. The SLP will work closely with patients and their families to develop and implement individualized treatment plans, ensuring the highest quality of care and improved patient outcomes.

Key Responsibilities

• Develop and implement individualized treatment plans based on assessment results.
• Provide direct therapy services to patients in their homes, focusing on improving communication and swallowing skills.
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive patient care.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.

Required Skills

• Active Oregon SLP License.
• Experience with Oasis Documentation.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
