Job Overview

We are seeking a dedicated and passionate Home Health Speech Language Pathologist (SLP) to provide essential therapy services to patients in their homes. This role offers the opportunity to improve the quality of life for individuals requiring speech and language rehabilitation in a comfortable and familiar environment.

Key Responsibilities

• Conduct comprehensive speech and language assessments to determine patient needs.
• Develop and implement individualized treatment plans to address communication and swallowing disorders.
• Provide direct therapy services to patients in their homes, monitoring progress and adjusting treatment as needed.
• Collaborate with other healthcare professionals to ensure coordinated and effective patient care.
• Maintain accurate and timely documentation of patient evaluations, treatment plans, and progress notes.

Required Skills

• Active Oregon SLP License
• Experience with Oasis Documentation
• Strong understanding of speech and language disorders
• Excellent communication and interpersonal skills
